NASPGHAN 2021 Nutritional Highlights

The North American Society for Pediatric Gastroenterology Hepatology and Nutrition (NASPGHAN) 2021 conference was held virtually December 12-18, 2021. The following are some of the nutritional highlights.

Quality Improvement Strategies Lead to Increased Utilization of Exclusive Enteral Nutrition for the Induction of Remission of Pediatric Crohn's Disease              

                     

             

 

 

Exclusive enteral nutrition (EEN) is an underutilized treatment to induce remission in pediatric Crohn's disease. Jenna Dowhaniuk, MD and Jillian Owens, RD from McMaster University present a poster "Quality Improvement Strategies lead to increased utilization of exclusive enteral nutrition for the induction of remission of pediatric Crohn's disease" of a QI project streamlining the process resulting in 3 fold increase in utilization of EEN while decreasing use of steroids. The program was well received by patients and parents who welcomed and appreciated the opportunity to use EEN for induction of remission.

           

   

The Role of Fiber During Nutritional Management of Children with Intestinal Failure
Anna Tuttle MS, RD, LDN, CNSC (LeBonheur Children's Hospital)
presented on risk factors for vitamin and mineral deficiencies in short bowel syndrome, factors to consider during monitoring of micronutrient status, and management options for replenishment and maintenance therapy.

Brad Pasternak, MD (Phoenix Children's Hospital) presents on the Perioperative Management of Pediatric Crohn's Disease where Enhanced Recovery Programs (ERPs), which include use of enteral nutrition, are shown to improve patient post-op outcomes.

Integrative and Culinary Medicine for the GI patient – Ann Ming Yeh, MD Lucile Packard Stanford Children's Health
Dr. Ann Ming Yeh presents the role of integrative medicine including nutrition as "culinary medicine". She describes teaching kitchens as an important part of patient education. Cooking videos made here to aid patients in learning to make dietary changes were shown to increase adherence to the Specific Carbohydrate Diet. To learn more visit scrumptious.stanfordchildrens.org

Therapeutic Diets: When Food is Medicine
Natalie Stoner, RD, CSP, LDN (Children's Hospital of Philadelphia)
presented on Evaluating the Use of Enteral Nutrition Therapy to Improve Disease Outcomes in Children with VEO-IBD. Patients with VEO-IBD are at increased risk of malnourishment and growth failure. EEN (80%) in combination with an anti-inflammatory diet for 12 weeks resolved malnutrition in most patients. Ongoing research will help determine which patients may benefit the most from EEN to improve both malnutrition and disease status.

Why Do We Need Dietary Therapies for IBD

Dr. Lindsey Albenberg, a clinician and researcher from Children's Hospital of Philadelphia, describes the rapidly increasing incidence of IBD and its relationship to diet, microbiome and the immune system. She reviews the rationale and science supporting the use of dietary therapy to compliment drug therapy as an avenue to potentially achieve higher, more sustainable and possibly safer levels of remission long term in pediatric patients.

The Crohn's Disease Exclusion Diet Updates: December 2021

Rotem Sigall Boneh, RD, a primary researcher and developer of CDED, provides an overview of the accumulating data with CDED in combination with PEN, including the newly published results of adult data with important endoscopic findings and further shares real world experience and application of nutritional therapy.

IBD Anti-inflammatory Diet or IBD-AID: Proof of Concept

Dr. Ana Maldonado-Contreras, a lead researcher in IBD-AID explains the relationship between diet, microbiome and immune function with the design and rational of IBD-AID to manipulate the microbiome. She shares the recently published data of the impact of IBD-AID on the microbiome and cytokine levels specific to food components.

Nutritional Therapy: Perioperative + Complicated Crohn's Disease

At the NTforIBD Nutritional Symposium prepared for NASPGHAN2021, Professor Day provides insight into the important role of EEN, an underutilized option to both induce remission and improve outcomes in complicated and peri-operative patients.
